Before diving everybody needs to fill in and sign some participant forms for medical and liability issues. Personal data are not used for any purposes and only kept as a confidential diver record for the time national law and regulations dictate. All forms comply with EU regulations and RSTC standards.
If you are a certified diver you need to fill in and sign the Certified/Student Diver Liability Release and Medical form and Standard Safe Diving and Health Declaration form.
If you plan to participate in a discovery Scuba Diving or Snorkelling program you need to fill in and sign the Scuba Diving Experience/ Snorkelling Liability Release and Medical form.
If the person diving is under age, a signature from the parent or guardian is compulsory.
If you are planning to do enrol in any Diving Course you need to fill in the Certified/Student Diver Liability Release and Medical form and Standard Safe Diving and Health Declaration form.  For some of the medical statements, you may be asked  a note from a doctor to enable you to dive.

You can print and sign the forms or we can provide you with them when you come at the dive centre.
